Random Urine as Screening Test for Microalbuminuria in Diabetic patients

Abstract
Objectives:
@EN Diabetic nephropathy is a major cause of morbidity and mortality in diabetic subjects, which may ultimately affect 40 to 50% presence of microalbuminuria in type-1 diabetic patients, predicts the later development of end-stage renal disease,
and in
type-2 diabetic patients, early death from cardiovascular causes. The risk of developing clinical nephropathy is approximately 20 times greater in patients with microalbuminuria. For the early diagnosis of diabetic nephropathy, it is best to use
the
albumin excretion rate (AER). However, it is a bothering test to perform in the outpatient setting, and it is cometimes affected by inaccurate urine collection. So for the early detection of microalbuminuria, we conducted a study with
Micro-Bumintest
compard with AER (24 hour and random urine, RIA) and corrected albumin/creatinine ratio( 24 hour and random urine).
@ES Methods:
@EN We measured quantitative AER from 24 hour and random urine and AER was compared with semiquantitative Microbumintest. In the rancom and 24 hour urine, corrected aib/creatinine ratio was measured simultaneously.
@ES Results:
@EN 1) Sensitivity of Micro-Bumintest which examined simultaneously with RIA from random urine was over all 80% (Table 2, 4).
2) Sensitivity of random urine RIA compared with AER was 91%, which was best result of the test done.
3) Both random urine RIA and A/C ratio were well correlate with AER statistically(Fig. 1, 2).
4) In the range of 20~50§¶/ml of AER, both RIA and Micro-Bumintest show somewhat low sensitivity and positive predictability. So, we must pay attention to this range microalbuminuria.
@ES Conclusion:
@EN For screening test of microalbuminuria, Micro-Bumintest, RIA Albumin/Creatinine ratio in random urine are simple easy methods in out-patient setting, which significantly correlate with 24 hour urine AER. But, in the range of 20~50§¶/ml
micro-albuminuria, we must pay attention to presence of microalbuminuria.
